Take the below assessment, and you will be advised as to what to do next. Self-assessment | Ontario.ca The self-assessment is meant as an aid and cannot diagnose you. Anyone who is sick or has any symptoms of illness, including those not listed in this self-assessment tool, should stay home and seek assessment from a health care provider if needed. All individuals are advised to wear a mask in crowded indoor public spaces, especially those at higher risk. You can also take it on behalf of a student, child, or someone else. Here's what you need to know: Please note that not all visits will be covered by your insurance provider. Unsure if your insurance will cover the visit? No worries! Reach out to DCSA/ OTSU for assistance. If it's confirmed that your insurance won't cover the visit, you'll be responsible for covering the fee at the time of your appointment. Authorization for Release of Medical Records A form is available to be picked up and signed from the Campus Health and Wellness Centre to release necessary medical information to a specific recipient. Please note there is a fee that must be paid before the request is processed. Completed and signed forms can be submitted in person, faxed or emailed with proof of identification. Confidentiality When booking an appointment, students are asked the nature of the visit, so that an appropriate amount of time can be booked for that appointment. For example, mental health and periodic health visits (PHV) require longer appointment times. Only general information is required to assign the appropriate amount of time for your appointment. You do not need to disclose the entire nature of your visit to the reception staff. All services provided at Campus Health and Wellness Centre are confidential. If you request a student medical certificate/sick note for academic consideration, only the information agreed upon by you and your doctor will be included on the form. No one from an academic or administrative department has access to your health record unless you’ve granted permission. Parents, guardians and other family members cannot have access to any information about you, including whether or not you have ever visited the Campus Health and Wellness Centre, without your expressed or written consent. Personal Health Information Protection Act, 2004 (PHIPA) is Ontario’s new health-specific privacy legislation. PHIPA governs the manner in which personal health information may be collected, used and disclosed within the health care system.
