Failure to register and submit all completed required documentation by the deadline date above will result in you not being eligible for placement for the current term.

Note: Please be advised that Field Placements are available to Ontario residents only.

Field placement instructions

Please complete the following steps once you have registered for your field placement or clinical course:

  • Select, print and thoroughly review the Field Placement Application Package that pertains to your program of study. Follow all directions and submit all required documentation as indicated in this document by the submission deadlines above.
  • Select, print and complete the Entry Immunization Form (required for all field placements and clinicals).

Criminal Reference Check Requirement

The required Criminal Reference Check with Vulnerable Sector Screen must be valid within 6 months of the placement start date.

If you live in the Durham Region, you are required to complete the Durham Regional Police Criminal Reference Check request form (DRP CRC) to obtain this check.  The DRPS CRC request form can also be found in each of the placement application documents.

If you live outside of the durham region, please follow the instructions of your local police detachment.  If your detachment requires a signature from Durham College, you will need to obtain the application form from your local police department or their website and then book an appointment with one of Durham College Insurance and Risk Management department’s signing officers, Elaine Wannamaker or John Neil.
